How the Referral Process Works
Step 1 Referral Received
Submit a referral by phone, email or through the referral form below.
We review the client's requirements, assessment information and funding pathway and confirm the next steps.
Step 2 Scope Review and Site Assessment
Many standard modifications can be quoted directly from an Occupational Therapist report using our schedule of rates.
Where site conditions may affect the installation approach, we arrange a site assessment to verify dimensions, substrates, access requirements and scope.
Any site visit requirements or applicable fees are discussed before attendance.
Step 3 Itemised Fixed-Price Quotation
Quotes are prepared in fixed-price, itemised format suitable for:
NDIS plan management
Support at Home AT-HM funding
DVA-funded works
Private clients
All quotations clearly identify any travel or mobilisation costs where applicable.
Step 4 Scheduling and Installation
Works are scheduled in consultation with the participant and their support network.
All modifications are completed in accordance with relevant Australian Standards, including AS1428.1 where applicable.
Step 5 Completion and Documentation
On completion, we provide:
Invoice
Completion photographs
Written confirmation of works completed
The referring Occupational Therapist, Support Coordinator or Care Manager is notified promptly.

Scope of Works
HHM specialises exclusively in minor, non-structural home modifications as defined under the NDIS minor modification framework and the Support at Home AT-HM Scheme.
We do not carry out:
Structural building works
Plumbing works
Electrical works
Works requiring builder registration or building permits
Where a client's needs fall outside our scope, we will advise this clearly at the time of enquiry and assist with referral pathways where possible.
Funding Pathways
NDIS
We accept referrals for plan-managed and self-managed participants.
Quotes are itemised to support plan management and approval processes.
Please note: Harbour Home Modifications is not a registered NDIS provider. Agency-managed participants should confirm suitability with their support coordinator prior to engaging.
Support at Home AT-HM Scheme
We provide fixed-price quotations and completion documentation suitable for AT-HM funded modifications across applicable funding tiers.
DVA Rehabilitation Appliances Program
HHM can assist with home modification referrals relating to DVA-funded works. Contact us to discuss a specific referral and funding pathway.
Private Clients
Private clients are welcome and receive the same fixed-price quotation and documentation process.
About HHM
Harbour Home Modifications is owned and operated by Ben Thorn, a qualified carpenter with 20 years of experience across residential construction, commercial projects, project management and accessibility-related works.
Relevant experience includes patient safety refurbishments across multiple Perth metropolitan hospitals.
Ben holds a Diploma of Project Management and Certificate III in Carpentry and Joinery and manages every referral personally, from initial enquiry through to completion.
HHM specialises exclusively in minor, non-structural home modifications for NDIS participants, Support at Home clients, DVA clients and private clients throughout Albany and the Great Southern.
HHM operates under the NDIS Code of Conduct and the Aged Care Code of Conduct and maintains appropriate public liability insurance and documented safe work systems for occupied residential environments.
